Giving up your pet online form

We know what a difficult decision it is to handover a pet, by filling out this form you will help us understand your pet and their needs. Once you submit your details we aim to be in touch within 72 hours. If you are handing over a dog or horse, it is likely that our team will arrange an assessment prior to intake.

Please use this form for dogs, cats and horses. If you are looking to handover another pet, please email us on reception@oaktreeanimals.org.uk with details of your pet and we will be in touch.

Please be completely open and honest about your situation - the more we know, the easier it is for us to understand the best way to proceed. Please remember that we are not here to judge or criticise you, we are here to help you and your pet.
